To help us accurately evaluate you or your child's dietary patterns and nutritional status please provide us with a detailed seven-day record of the patient/client entire food and beverage intake. 

It is Important and a must to get and Bring Your Referral From Your Doctor....

Your First Visit:
The first appointment takes 45 minutes.
Weight and height measurements are obtained at time of visit. The seven-day food diary & intake record will be evaluated and a written nutrition plan is developed specifically for the patient/client.  Follow-up appointments are 20 minutes in duration. 

What is covered at your First Visit: 
An appointment in our office consists of an initial 45 minute consultation which includes a review of: medical history, anthropometrics data, intake of seven day food diary, resulting in the development of a medical nutrition therapy plan.
 
The patient will be extensively counseled on implementing the nutrition plan.

Follow-up appointments are scheduled to monitor progress. Timely reports detailing progress are sent to the referring physician.

The nutrition counseling component to the visit is based on a behavior modification approach to change problem eating patterns and incorporates lifestyle changes including exercise.

We do not recommend “diets”, but instead emphasize healthy eating plans.
 
As health professionals, our counseling is conducted in a diplomatic and sympathetic manner. Children are especially instructed in a kind, positive manner with emphasis on changing eating and exercise patterns. We do not focus on weight issues for appearance but for the purpose of achieving and maintaining good health.

Patients leave our office with a complete nutrition and healthy eating plan specifically designed for their lifestyle and eating challenges. Educational material is provided to assist them in meeting their goals.

On Time Policy / Cancellations:



The office is proud of our on time performance. If you cancel an appointment with less than 48 hours notice a rescheduling fee  will be imposed since your time slot cannot be filled by another patient. It is understandable that appointments will occasionally need to be changed with short notice due to emergencies or other compelling circumstances. Late arrivals may not be able seen.

Insurance Participation:  
We participate with the following insurance plans. You must check with your individual insurance plan to determine coverage for nutrition services.

The following list includes the requirements of each insurance plan in regard to authorizations.
Unfortunately most insurance plans do not cover WEIGHT MANAGEMENT.  

 When it comes to nutrition services the following rules apply:
An HMO requires a referral with an authorization number. 
A POS requires a call to insurance carrier to determine if this is a covered benefit. 
A PPO requires PCP script 

All referrals required are to be obtained from your Primary Care Physicians.

All referrals must be at our office or in hand at the time of confirmation or visits will be subject to cancellation. A referral is based solely on medical necessary and is not a guarantee of payment. Please check with the insurance carrier to make sure this is a covered benefit for the specified diagnosis.
Question:
My insurance does not require referrals do I still need a referral to come to you? Yes.  Even if your insurance company does not require referrals to speciality physicians our services fall under ancillary providers and education services which ALWAYS need referral or a physician prescription regardless of the insurance plan.  

Family Nutrition Center participates with the following insurance plans:
  • AMERIGROUP --No longer covers nutrition services as of January 2011.
  • Aetna HMO requires a referral with an authorization number.  Aetna DOES NOT COVER WEIGHT MANAGEMENT.
  • Aetna PPO requires a prescription from your primary doctor.  MAY NOT cover weight management depends on group plan. 
  • BLUE CROSS/BLUE SHIELD HEALTH OPTIONS requires a referral with authorization number. Blue Cross/Blue Shield PPO plans DO NOT cover nutrition services
  • CIGNA PPO/HMO requires a script from the referring MD with the number of visits and an expiration date.  HMO requires a referral.  PPO DOES NOT  cover weight management.  
  • HEALTHEASE/WellCare and Staywell These insurances only cover Diabetes and Renal Disease.  Under special circumstances MD can obtain an authorization number with approval from insurance company.
  • MEDICARE requires a script for PCP. The only diagnosis covered are diabetes and renal failure. Supplement insurance will be accepted only if it is a rollover from Medicare. We do not bill third party. Patient must pay out of pocket otherwise.
  • Summit/Coventry  requires a referral from PCP no referral number is required
  • MEDICA requires a referral with an authorization number.  A script alone can not be accepted
